Health Insurance Claim Rejected? Complaint Steps in India
Short answer: If a health insurance claim is rejected, reduced or delayed, first ask the insurer for the decision and policy clause in writing. Compare that reason with your policy schedule, wording, proposal disclosures and medical records. Submit a documented grievance to the insurer, keep its complaint reference, and escalate an unresolved or unsatisfactory complaint through IRDAI’s Bima Bharosa system. Depending on the dispute and eligibility, the Insurance Ombudsman or a consumer commission may also be available.

What changed and why this guide matters
IRDAI’s 2024 health-insurance and policyholder-protection framework placed renewed emphasis on clear communication, claims servicing and grievance redressal. The practical sequence for a policyholder remains important: obtain the insurer’s written position, complain to the insurer first, then use the regulator’s grievance platform or another eligible forum if the issue is not resolved. Bima Bharosa, previously known as the Integrated Grievance Management System, lets a policyholder register and track a complaint involving an insurer.
This does not mean that IRDAI itself adjudicates every disputed claim or orders payment merely because a complaint is filed. The grievance channel helps route, monitor and facilitate re-examination by the insurer. A contractual dispute may ultimately require the Insurance Ombudsman, consumer dispute redressal machinery or a court, depending on the amount, subject and facts.
Health insurance claim rejected: start with the written reason
Do not rely on a telephone summary such as “not covered” or “documents missing.” Ask for the final decision in writing, including the amount claimed, amount admitted, deductions, policy clause relied on and reasons for every disputed component. A cashless pre-authorisation refusal is not always the same as rejection of the final reimbursement claim. Cashless treatment is a payment arrangement; the underlying claim may still need to be submitted as reimbursement if the policy and circumstances allow it.
Check whether the communication says rejected, repudiated, closed, deficient, pending, partly allowed or subject to further documents. Each status needs a different response. A deficiency can sometimes be cured with a discharge summary, diagnostic report or itemised bill. A repudiation normally calls for a reasoned challenge against the cited clause and facts.
Documents to collect before complaining
- Policy schedule, complete policy wording and customer information sheet for the relevant year.
- Proposal form or recorded proposal answers, endorsements and renewal history.
- Claim form, cashless request, pre-authorisation response and claim correspondence.
- Hospital admission notes, discharge summary, prescriptions, investigation reports and treating-doctor clarification where relevant.
- Itemised hospital bill, receipts and proof of payment.
- The insurer or third-party administrator’s rejection, deduction or query letter.
- Emails, portal screenshots and call-reference numbers arranged by date.
- Identity, bank and relationship documents only where legitimately required.
Keep originals safely. Send clear copies through an official channel and preserve delivery evidence. Do not send OTPs, card PINs or remote-screen access. A grievance portal does not require payment to release a claim. Similar verification discipline is useful when dealing with other financial-service complaints; our RBI Ombudsman complaint guide explains why the first complaint, reference number and chronology matter in another regulated sector.
Step 1: compare the reason with the policy
Find the exact definition, exclusion, waiting period, sub-limit, co-payment or claim condition quoted by the insurer. Read the surrounding provisions, not one isolated sentence. Confirm which policy year applies and whether an endorsement changed the terms. If the insurer alleges non-disclosure, compare the allegation with the questions actually asked in the proposal and the answers recorded.
For a deduction, separate genuinely non-payable items from medical expenses you believe fall within cover. Ask for a line-by-line calculation. For a waiting-period dispute, confirm the first policy inception date, continuity credits and whether the diagnosis fits the defined condition. For a “not medically necessary” decision, seek a concise clinical explanation from the treating doctor rather than an emotional appeal.
Step 2: submit a formal grievance to the insurer
Use the insurer’s grievance officer, official email, branch or online complaint page listed in the policy or on the insurer’s website. Put “formal grievance” and the policy or claim reference in the subject. State the requested remedy precisely: reconsider the rejection, admit a specified amount, explain deductions, correct claim status or provide a speaking decision.
Your complaint should contain a short chronology, the disputed decision, the policy clauses you rely on, documents attached and the relief requested. Avoid sending an unstructured bundle without an index. Ask for the complaint registration number and expected response date. IRDAI’s consumer information says that when a complaint is not resolved within two weeks, is unattended, or the response is unsatisfactory, the policyholder may escalate it to IRDAI.
Step 3: escalate through IRDAI Bima Bharosa
Use the official Bima Bharosa portal. The platform states that it does not ask complainants for payments and warns against scanning QR codes or using links that claim to receive disbursal. For a claim complaint, keep the policy, certificate or claim identifier ready. The registration form accepts a complaint description and supported document formats subject to portal limits.
You may also use the IRDAI Grievance Call Centre channels listed on the regulator’s grievance pages. Verify current numbers and addresses on IRDAI before acting because contact details can change. Save the Bima Bharosa token or reference and track the complaint. Do not create multiple complaints for the same issue unless the portal or regulator instructs you to do so; duplicate references can fragment the record.
Step 4: consider the Insurance Ombudsman
The Insurance Ombudsman is separate from Bima Bharosa. It can consider eligible complaints within its jurisdiction under the applicable Ombudsman rules, including certain claim disputes. Before filing, verify current monetary jurisdiction, territorial office, eligible complainant, subject coverage and timing on the Council for Insurance Ombudsmen’s official site.
Prepare the insurer’s final response, your original grievance, policy, claim papers and evidence. Explain what remains unresolved and the remedy sought. Do not assume that a Bima Bharosa registration automatically becomes an Ombudsman complaint. Each channel has its own filing process and eligibility conditions.
Common health claim rejection reasons and useful responses
Waiting period or exclusion
Ask the insurer to identify the condition, applicable waiting period and dates used. Check continuity after portability or renewal and any endorsement. If the diagnosis differs from the excluded condition, obtain a medical clarification.
Non-disclosure or misrepresentation
Request the proposal record and the precise undisclosed fact alleged. Compare it with the questions asked, records available at proposal and the medical relevance claimed. Do not alter or fabricate history; correct factual errors with authentic records.
Hospital or treatment eligibility
Confirm whether the dispute concerns the hospital definition, day-care treatment, domiciliary care, experimental treatment or a network-only cashless issue. A non-network hospital may affect cashless service without automatically deciding reimbursement eligibility.
Late intimation or late submission
Explain the reason with evidence, such as an emergency or inability to obtain records. Ask whether the policy allows condonation where delay was beyond the claimant’s control. Submit the claim without further delay.
Missing or inconsistent documents
Answer every query in an indexed response. If the hospital record contains an error, ask the hospital for a signed correction or clarification; never modify a medical document yourself.
Worked example
Meera’s reimbursement claim is rejected as a pre-existing disease within a waiting period. She asks for the exact clause, proposal answers and medical basis. Her earlier records show no diagnosis before policy inception, while the discharge summary uses a phrase that could imply a longer history. She obtains a factual letter from the treating specialist explaining when the condition was first diagnosed, submits a formal grievance with a dated index and asks the insurer to reconsider. If the written response remains unsatisfactory after the insurer’s grievance process, she can register the unresolved complaint on Bima Bharosa and check Ombudsman eligibility.
The example does not guarantee payment. It shows how a focused evidentiary response is stronger than repeatedly saying the rejection is unfair.
What does not change
A grievance does not rewrite the policy, make excluded expenses payable or guarantee full settlement. Policyholders must still give truthful information and reasonable documents. Insurers must still assess claims under the contract and regulatory framework. Cashless authorisation remains distinct from the final admissibility decision. Filing a regulatory grievance also does not pause every contractual or statutory deadline automatically.
Action checklist
- Get the rejection or deduction reason in writing.
- Identify the exact policy clause and policy year.
- Build a dated document index and retain originals.
- Submit a precise formal grievance to the insurer.
- Keep the complaint number and response.
- Escalate an unresolved complaint through official Bima Bharosa channels.
- Check Insurance Ombudsman or consumer-forum eligibility without missing time limits.
- Reject anyone asking for payment, QR scanning or an OTP to “release” the claim.
Frequently asked questions
Can I complain directly to IRDAI before contacting the insurer?
Use the insurer’s grievance mechanism first and keep its complaint reference. IRDAI’s escalation process is designed for unresolved, unattended or unsatisfactory insurer complaints.
Is cashless rejection the end of my claim?
Not always. Cashless service and reimbursement eligibility are different questions. Ask the insurer and hospital what route remains available under the policy.
Does Bima Bharosa charge a fee?
The official portal warns that it does not ask complainants for any payment. Do not scan a QR code or pay a person promising disbursal.
How long should I wait for the insurer?
IRDAI’s grievance information states that an unresolved complaint may be escalated if it is not resolved within two weeks, is unattended or is not resolved to your satisfaction. Verify the current rule and preserve the receipt date.
Can an agent file for me?
You may take assistance, but verify who is authorised, do not share OTPs or passwords, and ensure every fact filed in your name is accurate.
Should I also report suspected claim scams?
Yes, preserve evidence and use the insurer’s verified fraud channel and appropriate cybercrime or police channel. Our digital-arrest scam fact check explains common pressure tactics, while the UPI refund complaint guide shows how to document payment problems.
Conclusion
When a health insurance claim is rejected, the strongest response is a documented sequence: obtain the written reason, test it against the policy and authentic medical records, complain formally to the insurer, then escalate through Bima Bharosa or an eligible adjudicatory forum. Keep the chronology short, the evidence organised and every submission truthful.
